In order to create a prescription, you will need to
know the protocol number (to see the list click here),
and you will need to know the patient height (in cm or
inches) and the patient weight (kg or lbs). We will
calculate the surface area according to the standard
formula of DuBois and DuBois (and limit it to 2.2m2)
and calculate the appropriate
dose (rounded up or down appropriately).
Our scripts will calculate
the creatinine clearance for your information - but you
have to provide us with the patient age (yrs), sex and
creatinine (umol).

(males)= 1.25 x (140-age) x wt(kgs) / creatinine(umol)
Creatinine clearance
(females)= 1.05 x (140-age) x wt(kgs) / creatinine(umol)
Some of the support medications will be included (if we
recognize your email address from your registration data,
we will use your preferences, otherwise we will use our
defaults). After we have created your script, we
will email it back to you as a WORD or WordPerfect
attachment so you will be free to edit them as you wish. This
will take a few minutes. (If you give an NHSnet
email address, there may be a slight delay as incoming
emails are screened through a firewall).
